When You're Trying, and It's Taking Longer Than Expected

If you've been trying to conceive for a while without success, you're not alone — and you don't have to figure out the next step by yourself. Many couples in Bengaluru come to us feeling anxious, unsure whether the delay is "normal" or something to act on. A simple guideline: if you're under 35 and have been trying for over a year, or over 35 and trying for over six months, it's a good time to get evaluated. If you already know of a condition like PCOS, endometriosis, or irregular cycles, don't wait that long — come in sooner.

What a Fertility Evaluation Actually Involves

We start with a conversation, not a battery of tests. Based on your history, evaluation typically includes:

  • A review of your cycle, past pregnancies, and any relevant surgeries or conditions

  • Hormone blood tests

  • A pelvic ultrasound to check the uterus and ovaries

  • Semen analysis for your partner

  • In some cases, a tubal patency test to check the fallopian tubes are open

Treatment, Matched to Your Situation

Fertility care isn't one-size-fits-all. Depending on what the evaluation shows, options include:

  • Lifestyle and cycle-tracking guidance — often the first step, and sometimes enough on its own

  • Ovulation induction with medication, monitored closely with ultrasound

  • IUI (Intrauterine Insemination) — a well-established, minimally invasive first-line treatment for mild male-factor or unexplained infertility, which I perform regularly at both my hospitals

  • Surgical correction where a structural issue (like a fibroid, polyp, or blocked tube) is the cause

  • Referral for IVF/ICSI when more advanced treatment is needed, coordinated with trusted fertility centres
